Accreditation Instruments
These accreditation instruments are completed by health services being surveyed. The first table contains instruments for completion by hospitals or health services with multiple hospitals and the second table contains instruments for completion by General Practices.

Hospital/Health Service Accreditation Instruments

This instrument is completed by hospitals or health services with multiple sites (hospitals). The health service provides background information on: any changes which have impacted on the intern/PGY2 program since the last survey visit; facilities available to the interns/PGY2s; the relationship between the parent and rotational health service; and a breakdown of intern and PGY2 positions at the health service.

This instrument is completed by hospitals or health services with multiple sites (hospitals) prior to the survey visit and by the survey team during the survey visit. It is used as a framework for assessing compliance with the accreditation functions and standards.

This instrument is completed by hospitals or health services with multiple sites (hospitals) who rotate PGY2s to other health services or sites which are not being surveyed during the visit.

This instrument is completed by General Practices. The General Practice provides background information on: any changes which have impacted on the intern/PGY2 program since the last survey visit; facilities available to the interns/PGY2s; the relationship between the parent and rotational health service; the orientation program, supervision provided etc.
